Officers and Advisors Report
Due within 1 week of officer elections

Hazing Compliance Form
Due at chapter's meeting with Director to review recruitment/intake program

Chapter Anti-Hazing Statement
Due 1 week after the end of Recruitment/Intake

Social Event Notification Form
Due 1 week prior to any event at which alcohol and/or outside guests will be present

Notice of Recruitment Form
Due before any recruitment activities can occur

Notice of Intake Form
Due before any intake activities can occur

Verification of New Members Form
IFC/PHA Chapters - Due within 5 business days of bid acceptance

Verification of Aspirants Form
NPHC Chapters - Due immediately after interest meeting, before official process begins

Due 2 weeks in advance
